Certified Community Behavioral Health Clinic (CCBHC)

About

As a Texas Certified Community Behavioral Health Clinic (CCBHC), SCAN focuses on treating the whole person through access to integrated, evidence-based mental health and substance use services, and primary care screenings. SCAN meets stringent criteria regarding timeliness of access, quality reporting, staffing, and coordination with social services, criminal justice, and education systems.

The CCBHC model integrates primary care screenings and substance use disorder care into mental health care settings clinically, financially, and administratively, with the goal of improving overall health outcomes.

The CCBHC model is comprised of the following services, which SCAN provides directly or has arranged with other community agencies to provide them:

  • Crisis mental health services, including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization
  • Screening, assessment, and diagnosis, including risk management
  • Patient-centered treatment planning
  • Comprehensive outpatient mental health and substance use services
  • Outpatient primary care screening and monitoring of key health indicators and health risk
  • Targeted-case management
  • Psychiatric rehabilitation services, defined in Texas as mental health rehabilitation
  • Development of comprehensive community recovery supports, including peer support, counseling services, and family support services

CCBHCs must deliver services to members of the armed services and veterans.
